Volunteer Registration
Manager and LRM registration is live for the season. See the info to the right or visit the Locker Room Monitor page on what is needed to register and help with Polar Caps for the 2026-2027 season.
USA Hockey Requirement
Each volunteer must register first with USA Hockey and then return to this Registration Session to sign up for the Season.
Step 1: CLICK HERE to go to USA Hockey Registration and register yourself. Be sure to PRINT and SAVE each confirmation code(s). Each volunteer must have their own unique USA Hockey confirmation number.
Step 2: Return to this registration and proceed. Later in this registration session you will need to enter the USA Hockey Confirmation Code that you received in Step 1. It takes a few minutes (sometimes even longer) for the USA Hockey database to update, so your confirmation code may not work in our system right away after they issue it.
Additional Requirements
Once you have completed your USA Hockey and West Madison Polar Caps registration, you need to complete the remaining requirements through USA Hockey including:

Before being able to supervise a locker room or serve as a team manager or board member, a background check and SafeSport must be complete. Reach out to the registrar with questions.
